Does a shed on a slab leave the slab behind?

Usually, unless you ask. Breaking out a concrete pad is a different job with heavier equipment. Say whether the pad stays or goes when you call.

Does my HOA need to know?

Removing a shed rarely needs approval, but many Spring deed restrictions govern what goes up afterward. If you plan to replace it, check with your architectural committee first.

Can a playset be reused instead of hauled?

Sometimes. A sound wooden set can find a new yard, but a set with soft or split beams is a hazard to pass on. Ask the contractor whether they donate usable pieces.
